Though I cannot address your specific condition without having examined you, what you describe is commonly seen in patients with excessive sweating of the feet, and is the result of the maceration of the tissues of the sole of the feet. Such a condition can generally be treated effectively by reduce the sweating.

I don't understand about your doctor's refusal to refer you to a specialist. Is that as part of his acting in a gatekeeper role for your insurer or is that some sort of independent, capricious or ego-driven decision on his part? If it is the former, certainly their are some procedures in place to challenge such a decision. If it is the later, and he hasn't been unable to help you, then it would seem to me, baring other facts not herein stated, that he is acting professionally irresponsible. If you or your insurance is paying for it anyhow, go see a podiatrist or a dermatologist.
