A crushing injury such as you describe can cause a host of damage other than a fracture. She might have suffered a bone contusion and also there are nerves, blood and lymphatic vessels, tendons, joint structures and muscles which might have also sustained severe injury. I don't know that it is very wise to delay seeing a doctor until some time next week, but general principles of initial care of trauma produced swelling, once a fracture has been ruled out, usually dictate elevation, rest (possibly the use of crutches) and the use of compression and cold therapy (after being given appropriate instructions and cautions for the use of both by a doctor). This certainly should have been discussed with you and your wife by the emergency room doctor. If appropriate pain medication had not been prescribed, I would contact the hospital for a prescription or recommendation for OTC analgesics. I see no sense in advising any exercises until her specific status is determined. In answer to your specific question about pain, a fracture would not necessarily produce more pain than the other situation which I enumerated and a fracture might be more easily and effectively treated. Get her some hands-on evaluation and care from a specialist.
