First of all, I would assume that along with the EMG you had a nerve conduction test. If not, I would think that you should have one as part of the diagnostic test for tarsal tunnel syndrome. In addition, there are some other physical signs, such as Tinel's, which is commonly positive in TTS. A positive with this sign is the radiation of electrical sensations and/or paresthesias when area of the tarsal tunnel is percussed. Unfortunately, TTS presents often with vague symptoms which are generally limited to the ankle. So, simply because a test is positive and you have symptoms, it does not necessarily mean that the symptoms present are the result of the condition which the test tends to indicate. But in the end, the diagnosis of tarsal tunnel syndrome is as much a clinical one as one based of tests, and diagnostic judgment is always necessary. It would seem logical that if the symptoms were exactly the same on both ankles and that the symptoms on one truly indicated TTS, that you would have TTS of both sides. But I would think that some further evaluation needs to be done, especially before any surgery is considered for tarsal tunnel syndrome on either side. On the other hand, if you are looking for me to suggest what else you might have or what an alternative diagnosis might be based simply on your stating that you have pain in your ankles, it would be impossible for me to do that in any meaningful way. Get a second opinion by hands-on evaluation.
