Chief,

Did you get any assistance with your situation yet? Yes, it is next to impossible as a Nuke forced conversion E-6 or above over ten years to get converted. I've been told I am one of 2 that the ECM's have seen happen. Very sorry to hear about your situation. I was also told to convert and then subsequently separate due to medication. I was saved through a string of stars aligning correctly.

On to your questions. The ISP is very easy and cut and dry. If you are on the ship/sub still get your CCC involved ALONG with personnel. Between the two entities, they should be able to figure it out for you. The documents you will need are a rough draft copy of your DD214, your "10 day notice", and a PG13 stating your willingness to either affiliate with the SELRES for 3 years, or IRR. If you have already done 17 years... and you are not being fully medically retired for your condition, PLEASE talk to a reserve recruiter about affiliating with the reserves since you only need 3 reserve years to retire. They may tell you no due to your back, but it's worth a shot.

The DD214... very important you have personnel triple check not only your character of discharge (honorable) but more importantly your "separation code". That code affects whether you are entitled to full, half, or no sep pay. As medical you should be entitled to full.

House hunting/job hunting, you unfortunately may have to sacrifice due to the short time frame you're working with.
If you have any further questions, need assistance etc. feel free to contact me. The documents for your ISP should be scanned and sent to ISP@NAVY.MIL
