I went to a podiatrist last year for a pair of diabetic shoes and he did not address them. They literal wear the lining out and get rubbed raw by all of my shoes. Now I have developed pain in the backs of my heels that feel like some kind of bruise. Any ideas? I could go to a different podiatrist, but am wondering if this is more approriate for an orthopedic specialist.

It may be typical Haglund's deformity or it may be osteoarthritic proliferation of bone at the insertion of the Achilles tendon.

I first attempt to have the patient use a heel pad in the sock liner of the shoe so as, if possible, to raise the bony protrusion above the counter of the shoe and eliminate the irritation. If it is Haglund's and this proves unsatisfactory, but the situation warrants, I generally suggest surgical revision of the bony protrusion of the heel. I always require no weight-bearing on the operated foot for several weeks after surgery, as Achilles tendonitis is not uncommon post-surgically.

Osteoarthritic proliferations require more serious surgery, as the area may be contiguous with the insertion of the Achilles tendon.
