Hi, your mother can sign up on NY state information registry. Free. https://www.health.ny.gov/vital_records/adoption.htm
this gives you the laws and rules, and the forms you need.
If you can't print it off, just write and request the appropriate form and they will mail it to you.

Did anyone confirm in the birth index the birth name.?
