I understand that in order for the LRP payment cycle to begin on my student loans, I must first provide a copy, complete with lender verification and signature, of my DD Form 2475 (DoD Educational LRP Annual Application) to the LRP Manager 90 days prior to my anniversary date. I will fax the completed DD Form 2475 to CNRC (N5312) at (901) 874-9327. Incomplete or improperly filled out forms will not be accepted.
