A third degree sprain is a serious injury which may take many months from which to recover, even if optimally treated. With severe ankle sprains there is a high incidence of incomplete recovery to pre-injury status, and propensity for future sprains. Physical therapy of a type and at an appropriate time as deemed by your attending doctor is generally essential for as full as possible of a recovery. Swelling is a normal physiologic response to injury of tissues, but as you are in a cast, you must quickly report to your doctor any excessive tighten of the cast resultant form such swelling, or severe additional problems may ensue. But since, if you have been given a cast, you are obviously under the care of a doctor, or if treated in an ER were probably instructed to obtain followup care, questions such as you are asking here should be directed to your doctor or one who knows the the specifics of your case.
