None of your questions can be given any truly meaningful answers, as those answers depend both upon many facts and factors which you have not included in your question, and necessitate a hands-on evaluation of your foot which would include x-rays.
I would assume that the silicone space you mentioned was suggested by the first doctor who recommended bunion surgery merely as a temporary device to possibly help in slowing down the hallux valgus (deviation toward the 5th toe) of the great toe, although you fail to state that any such deviation has occurred. But I would assume that, as both doctors recommended osteotomy procedures, your problem is not of the simplest variety of bunion, and there is essentially always hallux valgus present in bunions requiring an osteotomy.
Though no one has a crystal ball, bunions tend to become progressively worse as time passes, and it is a judgment call as to whether even an asymptomatic bunion should be corrected. You fail to mention frank pain, only sensitivity, so I cannot opine as to whether treatment needs prompt attention or can be delayed.

Once you are satisfied that you have a quality surgeon, I suppose it would be best to discuss with him/her your needs, desires and reservations, and proceed from there.
