As I so often have to say, there is simply no substitute for a hands-on physical examination, and venues such as this should not be thought of or used as a substitute for a real diagnostic evaluation. The complaints of the patient are but a starting point in such a process. Al thought I highly doubt that the pain that was first in your heel actually moved to the top of the foot, it is entirely possible that another pain cropped up in different areas of the foot, possibly due to different causes. Though you ask for "any ideas," that is not really what you want, as ideas based on insufficient information are a dime-a-dozen, and are usually wrong and sometimes detrimental. What your want are responsible answers, and that can only be obtained through an actual examination by a qualified doctor. Once a diagnosis has been established, a site such as this can be helpful for further general information regrading that diagnosis and a general discussion of the pros and cons of treatments available.
