Assuming from what you have stated that the toe sensitivity is related to a neuropathy rather than a local cause, trauma or infection and not associated with gout, it is not likely that podiatric medicine can be of much value in your situation. Neuropathies are generally difficult problems to effectively treat, and many are not effectively treated, but I think that a neurologist would be the most appropriate choice of doctor. You have offered no information whereby an amputation could be anticipated, and as far as how great your disability will becom, that is anyone's guess at this time.
See a neurolgist for a detailed diagnosis and treatment if possible.
