Several reasons. First, the polish has the potential to flake off and could cause problems if it winds up in the wound. Secondly, though the color of the nailbed of the fingernails are more commonly checked for the purpose of observing your state of oxygenation during surgery, either by direct eye-balling or by a clip-on electronic device which makes the assessment, but the surgeon him/herself can check such status in the toenails if a tourniquet is not used and/or how quickly normal circulation returns after a tourniquet, if used, is released. Also, if your surgery requires some time in a recovery room, the color of the nail beds will likely be scrutinized by the nursing staff for the same purpose.
For many of the the same reasons, fingernail polish should not be used either.
