Unlike callouses on the ball of the feet which are most often resultant from inordinate and/or poorly dispersed pressure on areas below the metatarsal heads, calloused lesions of the heel are generally not bone related, and may not be totally pressure related either. You give no information as to its size or appearance, and offer no details as to the diagnosis previously made or the nature of the surgery which you have already undergone, but I assume that you had some sort of excision (full thickness or curettage) of a keratotic lesion. Hopefully a histological examination and identification was done. I have no idea of what "bringing the callous to the surface" means, as, by definition, a plantar callous is a surface lesion. Keratotic lesions have various possible specific diagnoses and etiologies, and any meaningful discussion of treatment or prognosis would depend on the precise nature of the lesion and its cause, if that can be determined. It may be that periodic reduction and perhaps padding might be your best option, but having not examined you, I certainly cannot offer that as the definitive answer which you would, of course, prefer.
